Please note that all passport applications that include a silhouette /description from the vet must be submitted on the up to date forms and must be sent “clean”, unfolded or marked otherwise a charge of £10 will be added to the passport fee for artwork charges as these are scanned directly into the passport and must be unmarked.
Please also note that applicants must be 18 years and over.
